At the beginning of the last line, Bb (at 'Moon') should be Dm, other than that great work!

Definitely F/A at the end of the first line. Musically it works there because it's in the middle of a thought - the F/A there gives the listener the idea that the singer is still finishing that thought. The F chord is used by itself works at the very end of the last line. This is virtually the exact same arrangement that Katharine Hepburn used in Breakfast at Tiffany's.

This works for the Sinatra version too if you put it in C (I couldn't find a good chord chart for that one on here).
