This uses a few chords remembered from seeing a transcription from Christian Korbanka somewhere but
that only applies to where the chords are 6's instead of just majors etc. The rest if my own devising.

I believe Eb6 is actually played as Cm7.
I watched the video of someone who plays it perfectly (https://www.youtube.com/watch?v=IEcYc8j1wHo)and from this video I believe the Gm to Eb6 part (bass walkthrough) goes something like this (roughly) :

The 2nd time just play an Eb chord x6534x instead of Cm and Cm7.
